한국에 치과위생사, 간호조무사 같이 치과에서 팀을 구성하는 직업들이 있듯이 호주에도 있습니다. 오늘은 간단히 이들 치과 관련 직업에 대해 이야기 해 보겠습니다. 하지만 이 역시 주마다 조금씩 차이가 있기 때문에 이 칼럼에서도 제가 근무하는 퀸즐랜드주 제도에 대해 이야기 할까 합니다.
호주에서는 치과의사와 함께 일하는 직업으로 치과위생사(Dental hygienist), 치과치료사(Dental Therapist) 그리고 Dental Hygienist와 Dental Therapist 자격증을 동시에 가지고 있는 Oral health therapist가 있습니다. 또 이들을 돕는 치과 보조사 (Dental Assistant)들도 있고요.
치과치료사(Dental Therapist)는 한국에는 없는 직업 같은데요, 호주 퀸즐랜드주에서는 1976년도부터 국립병원에서 일정기간 간단한 교육을 받고 자격증을 받아서 일하는 식으로 시작 되었습니다. 지금은 전문대학 2년 과정을 마쳐야 자격을 취득할 수 있도록 체계적으로 바뀌었죠.
Dental Therapist들은 치과의사의 지도 아래 5세부터 18세까지 아이들의 치과치료를 할 수 있어요. 엑스레이 촬영은 물론 유치에 한해 충치치료와 발치도 할 수 있고요.
반면에 치과위생사(Dental hygienist)는 한국의 치위생사와 많이 비슷한 것 같습니다. 이들 역시 치과의사의 지도가 필요하나, 모든 연령층의 환자들에게 예방진료(preventive treatment)를 할 수 있습니다. 이들은 스케일링, 불소도포, 치아미백 그리고 환자들에게 치아관리법 등을 교육시키는 일들 하죠.
하지만 지금은 Dental hygienist와 Dental Therapist 과정이 많이 사라지는 대신 Dental Therapist 와 Dental hygienist를 합친, 좀 더 전문적인 직업군인 Oral Health Therapist(OHT)가 늘어나는 추세에 있습니다.
OHT는 대학교 3년 과정을 이수해야 자격증을 취득할 수 있습니다. 이들은 단순히 치과의사를 돕는데 그치지않고 18세 미만 아이들의 치과치료를 치과의사처럼 할 수 있죠. 치과의사와의 차이점은 이들 역시 치과의사의 지도 아래 일할 수 있다는 점인데, 유치의 경우 치료와 발치를 모두 할 수 있지만, 영구치 충치치료는 아주 제한적으로만 할 수 있습니다.
또, 유치의 pulpectomy나 ledermix(항생제 성분의 드레싱 재료)도 사용이 제한되어 있으며, 유치 발치시에도 elevator는 사용할 수 없고, 약 처방전 또한 쓸 수 없습니다.
아래 내용은 치과치료사와 치과위생사의 업무범위(Scope of practice)를 간략하게 기술한 것입니다. 이들의 Scope of practice는 주마다 다르고, 또 자주 바뀌는 관계로 대략적인 내용만 적었습니다.

백문영은 2010년 호주 퀸즐랜드 치과 대학(University of Queensland)을 졸업하고, 2011년 호주 타운즈빌(Townsville)에서 치과의사로 일하다가 현재는 차터스 타워스(Charters Towers)에서 senior 치과의사로 3년째 근무하고 있다. 이곳에 거주하는 유일한 한국 사람이며, 고양이 두 마리와 함께 살고 있다. 저자가 사는 이곳부터 800km 내륙 까지는 치과의사가 없기 때문에 250km 떨어진 휴인던(Hughenden)과 400km 떨어져있는 리치몬드(Richmond)까지 맡고 있다.
